About this instrument

The WHO-5 is a short well-being questionnaire published by the World Health Organization. Higher scores mean better well-being over the past two weeks.

The WHO-5 Well-Being Index was developed by the World Health Organization and is the most widely used short measure of subjective well-being. Five items ask about cheerfulness, calm, energy, restful sleep and interest in daily life.

The raw total of 0-25 is multiplied by four to give a 0-100 percentage score. WHO guidance: below 50 suggests low well-being worth attention; below 28 suggests screening for depression (for example with the PHQ-9).

Unlike symptom scales, WHO-5 measures the presence of positive states. A low score is not a diagnosis, but persistently low well-being is an early signal worth noticing.
